Connected Communities Map
The purpose of the Connected Communities map is to showcase the organizations, agencies, programs and facilities that prioritize a person-centered approach and offer a continuum of care, thereby illuminating the "connected communities".
In the current Phase 1 of the map build, despite having locations plotted across the US, we primarily focused on three states—Arkansas, Massachusetts and Michigan—in order to showcase a sampling of how the data lays out, but also to understand the complexities of finding and plotting the data. As we continue to add to the data in these states and move into Phase 2, we will plot data in Georgia, Pennsylvania and Colorado.
